Rates

$225 per 50 minute session at your location. Office visits are currently only available to Kaiser Members at Kaiser's Sherman Terrace Office in Reseda.

Payment

Cash, check or credit card at time of service.

Insurance

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions of your insurer:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

A superbill will be provided if you wish to submit to your insurance company for reimbursement. I do not negotiate with nor submit claims to your insurance company.

Cancellation Policy

Insurance will not pay for missed or late canceled sessions. If you do not show up for your scheduled appointment or you cancel without notifying me at least 24 hours in advance, you will be required to pay the full cost of the session as booked.
